The Deeper 33, pattern 16: The contact form asks for a novel before a conversation.

Is your contact form asking for too much?

Published June 7, 2026 Updated October 7, 2026 By Rick Julian

How long should a therapy contact form be?

Every extra field is a small dare: prove you are serious enough to type your insurance ID before we reply. Many abandon instead. A short form—name, contact, one sentence—respects the courage it took to click Contact and leaves depth for humans.

Forms that feel like intake packets

Some sites request full histories, diagnoses, and partner details before a first reply. That belongs after consent, not in a web form.

Dropdowns with twenty insurance plans and mandatory attachments stall mobile users.

CAPTCHA puzzles and multi-page wizards signal bureaucracy before relationship.

Who never hits submit

Teens and young adults bail when forms look like homework. Parents researching at midnight close the tab when field count passes five.

High-functioning professionals want discretion—a novel in a contact box feels exposing.

Non-native speakers struggle with jargon labels like presenting concern versus what is bothering you.

Data you never needed pre-reply

Marketing teams sometimes copy medical intake templates onto public sites, confusing HIPAA boundaries and scaring visitors.

Insurance verification before a conversation can wait; clarity about whether you are in-network belongs on a fees page instead.

Spam tools can be lightweight—honeypots and rate limits—without torturing real humans.

Design the minimum viable ask

Collect name, email or phone, and a single open text box with a humane prompt.

Offer scheduling links beside the form for self-starters.

Send an auto-reply that sets response time expectations and links FAQs on fees and telehealth.

Questions therapists ask about this pattern

Will short forms increase spam?

Some. Use light technical filters and clear screening in your reply rather than front-loading the form.

Should I ask how they found me?

Optional one-line field is fine. Do not make marketing analytics the price of admission.

What about risk screening?

Public forms are not crisis assessments. State limits and offer crisis lines; conduct real screening live.

Is a portal better than a form?

For some practices, yes—if login is simple. If portal signup is harder than email, you lose leads.
